The \H disambiguate argument now shows on most navigation locations including:

article indices on -W, --web
tagged/incoming article lists on -W, --web and static
breadcrumb navigation on -W, --web and static

This makes it easier to understand what the topic is about when disambiguate is fundamental.

This change adds the "(mathematics)" disambiguate to "Classification (mathematics)", which makes the header much clearer than just "Classification".
